What your donation provides:

     $28 = one month of diabetic supplies for a patient
     $40 = one week of bus fare for patients and their escorts to appointments
     $90 = a nutritious lunch for everyone at Christ House
     $140 = one day of care from a nursing assistant

How to give:

1.  Click here: www.christhouse.org/donate
2.  Select to ‘Honor or remember someone with your gift’
3.  If you would like us to send a special, personalized card to the honoree (see images above), select ‘Notify the Recipient‘ and type in the address.
   •  You have the option to include an additional note in the “Include a note to the recipient” box. Or, simply type “none” in this box.
4.  Complete the rest of the donation form.
5.  For delivery of the Christmas card before December 25th, please donate by noon on Friday, December 15th
